PE Health Form:
The purpose of the health form is to inform coaches of specific health issues for each individual student. It is important for coaches to be aware of such issues to be prepared in case of an emergency. Please make sure that your child returns this form to their coach as soon as possible.
Grading System:
Students will be graded on a ten point grading scale (ex. 100 - 90 A). Points will be deducted each time a student: does not dress out or has unsatisfactory participation.
Grades will be calculated based on three main categories: participation, dressing out, & assessments.
Participation:

All policies of the GCSJH student handbook under student dress code shall apply. Dressing out for class is as follows:

The following types of assessments can be used this school year to assess students knowledge and understanding:

Parent Notes:
Students may be excused from participation for an illness by providing their teacher with a parent note . Parent notes should be given to the teacher at the beginning of the period and should include the student's name, parent's name, and a contact phone number or email address. The note should also state the reason for the student being unable to participate that day.
Extended time (no longer than 3 days) should be accompanied by a Doctor's Note. Students will be sent to the media center and provided with written assignments. Participation points will be given once the student completes the written assignment.
Locks and Locker Fees:
All students must have a lock and locker for PE use.

- Students must participate to earn daily credit for activities.
- If unable to participate for minor reasons a note from home will be acceptable. After 3 days a doctor's note will be required.
- Any student unable to participate for medical reasons will be sent to the media center to complete written assignments.
- The Presidential Physical Fitness Test will be given twice during the school year & will be graded upon completion of all areas and effort given.
- Students will be required to participate daily in PE. Partial points will be given to those students who participate while not dressed out. It is highly recommended that you stress to your child the importance of daily dress out and participation.
- Students wearing in appropriate footwear such as flip flops or sandals will be considered a safety concern and will be given an alternative activity for the day. They will also lose their full participation points for the day as well.
Dressing Out:
All policies of the GCSJH student handbook under student dress code shall apply. Dressing out for class is as follows:
- Students must wear a GCSJH issues PE uniform that is available through out department for $20 or $25. The uniform includes a gray t-shirt and green shorts. Please make sure that your child puts his/her name in the space provided on both garments. Last years uniform may also be worn. Students may also purchase more than one uniform if desired.
- Athletic shoes must be worn for all activities. No boots, flip flops, sandals, or backless shoes will be permitted for safety purposes.
- Students will NOT be allowed to share uniforms. Any student who violates this policy will be given a discipline referral.
- Warm-ups and sweat shirts/pants are permitted during the cold winter months.

- Performance task > Student performs a specific skill like passing a football
- Product task > Student provides a product such as a drawing of a specific skill
- Portfolio task > Student writes a short essay or creates a brochure on a certain topic
- Student Journals or Logs
- Individual or Group Projects
- Teacher Observation of Students Skill performed in a game like scenario
- Creating a personalized fitness plan > PYFP Worksheet

- The rental fee for a lock and locker is $5. A lost lock will cost $.4.50 to replace.
- Only school issued locks will be allowed for PE (Any other locks will be removed).
- Sharing of lockers and combinations is NEVER allowed. Violation of this policy will result in a discipline referral.
- The PE teachers will conduct random locker and uniform inspections. Students should not share lockers or uniforms.
- The PE department WILL NOT be responsible for lost or stolen articles and assumes NO responsibility for articles stored in lockers.
- Glass containers and aerosol sprays are not permitted in the locker rooms for safety reasons.
- No electronic devices will be allowed in the locker rooms. All electronic devices are strictly prohibited in the locker room.
